Fill out a name for your form and then click Create.
After creating your form, it will appear in the list of all Prequal forms. Click on your form to begin building it.
You are able to choose and customize which information you'd like your potential subcontractors to provide by clicking the blue toggle button. You can also click the Required checkbox to require a particular field.
If you'd like to preview your form before sending it out to your subcontractors, click the Preview button in the upper right corner.
When you are finished, selecting the fields to include, you are ready to invite your subcontractors to fill out the form to see if they prequalify
